Joanne Lee Sly is facing seven charges stemming from the fatal crash on the Kamilaroi Highway on April 2.
Sly, 48, from Karangi, appeared in Tamworth Local Court this week for the first time since she was charged by police from the Oxley Crash Investigation Unit, about a week after the crash.
Sly, who was represented by Legal Aid, was not required to enter pleas to the offences.
She is charged with dangerous driving occasioning death by driving in a manner dangerous, two counts of dangerous driving occasioning grievous bodily harm (GBH) by driving in a manner dangerous, two counts of causing bodily harm by misconduct when in charge of a motor vehicle, negligent driving occasioning death and negligent driving occasioning GBH.

Sly had her bail continued and will return to Tamworth court in July.
The crash happened about 6.45pm on April 2 on the Kamilaroi Highway, about 5km from Boggabri.
The southbound Toyota Camry collided with a northbound Honda Civic, carrying five people.
An 18-year-old Narrabri man, who was a backseat passenger in the northbound Civic, suffered critical injuries and died at the scene.
The driver, an 18-year-old woman, and two other passengers, a woman and man in their teens, were seriously injured. The driver of the Camry was also injured and taken to hospital.
