Jockey Greg Ryan and horse Son Of A Dun hit the decks when it collapsed at the 900 metre mark.
The domino-effect saw Josh Adams, Rachel King, Kath Bell-Pitomac and Wendy Peel take the fall along with Ryan.
Five jockeys were reportedly hospitalised, but a NSW Ambulance spokeswoman said only four riders were taken to Tamworth hospital.
Races were delayed for the day as ambulances tended to the injured.
It's believed Son Of A Dun, ridden by Greg Ryan suffered a heart attack and collapsed in race two at the Country Championships qualifier meeting on Sunday.
The fall resulted in a domino effect that forced the other riders out of the saddle.
The incident "subdued spirits" at the course, Tamworth Jockey Club interim chairman Wade Berryman said.
"No one likes to see horses fall, let alone have to be euthanised," he said.
"Certainly no one wants to see jockeys injured."
The on-course ambulance rushed to the scene after the dramatic fall dislodged the jockeys.
