Emergency services were called to Loftus Crescent in Homebush at 10.15am on Tuesday morning.
They found the 33-year-old man with chest and pelvic injuries after the 12-metre fall.
A police spokesman said the man had fallen three floors down the elevator shaft.
He was taken to Westmead Hospital in a serious condition.
It is understood the man was a builder at an apartment complex which was under construction.
