Emergency services were called to Maules Creek Road at Tarriaro, east of Narrabri, in the early hours of Saturday after reports of a single vehicle crash.
The five people travelling in the vehicle, four men and a woman, were trapped for several hours.
The 28-year-old man behind the wheel of the Toyota Landcruiser at the time was airlifted to John Hunter Hospital in Newcastle.

A 21-year-old man, who was the front seat passenger, was flown to John Hunter Hospital as well, where he remains in a critical but stable condition, according to police.
The three rear passengers, a 21-year-old woman and two men aged 20 and 22, also remain in hospital in a stable condition.
Officers from Oxley Police District are continuing to investigate the circumstances surrounding the crash.
A report will be prepared for the Coroner.

Five people injured in an early morning car crash on Saturday were trapped inside the vehicle for several hours after the accident.
Two young men have been flown to Newcastle's John Hunter Hospital with multiple injuries.
Emergency services were called to Maules Creek Road near Old Gunnedah Road at Tarriaro, about 28 kilometers east of Narrabri, just before 1.30am on Saturday morning following reports a Toyota Landcruiser had hit a wild pig.
After hitting the animal, the vehicle left the road and collided with a tree.

All five occupants of the Toyota - four men and a women - were trapped inside for several hours, according to NSW police.
They were released by Fire and Rescue NSW.
The driver - a 28-year-old man - was airlifted to Newcastle's John Hunter Hospital with multiple injuries in a serious condition.
The front seat passenger - a 21-year-old man - was taken by road ambulance to Narrabri Hospital before being airlifted to Newcastle.
A spokesperson for the the Westpac Rescue Helicopter reported he had suffered multiple limb fractures and other injuries, but was in a stable condition.
All three rear passengers, a 21-year-old woman and two men - aged 20 and 22 - sustained serious injuries.
The woman was airlifted to St George Hospital, while the older man was airlifted to John Hunter Hospital.
The younger man was taken by road ambulance to Narrabri Hospital.
Both the Tamworth and Newcastle Westpac Rescue Helicopters were tasked with responding to the car crash.
Officers attached to Oxley Police District established a crime scene and launched an investigation into the circumstances surrounding the crash.
Police inquiries continue.
Oxley police urged anyone with information, or dashcam footage from the area, is to come forward.
