The Gunnedah export, who played in the Swans’ grand final loss to the Bulldogs last season, was given every chance to prove his fitness this week.
But the 18-gamer was restricted to light duties during Thursday’s final training session and was ruled out.
A full update on his condition will be provided early next week.
Naismith, 24, featured in the opening three matches of 2017 before being sidelined for three rounds with a knee injury.
He returned two weeks ago in round seven, with the Swans snapping their six-game losing run in a 135-81 win over the Lions.
He backed up the following week in Sydney’s 120-78 defeat of North Melbourne but tweaked his medial ligament in the game.
It was the same injury that previously sidelined him.
