Following a public appeal for information after the early-morning collision on October 12, Tamworth police tracked down a 17-year-old girl.
She later presented at Tamworth Police Station where she was questioned by general duties officers.
The 17-year-old, who has a P-plate licence, was charged on Wednesday with negligent driving causing grievous bodily harm, not give particulars to injured person and fail to stop and assist after impact causing injury.
The teenage girl was issued a future court attendance notice to appear in Tamworth Local Court in December.
The impact of the crash seriously injured Jeannine Lewis.
Ms Lewis was on a morning cycle on Wahroonga Road, Hillvue, about 6.30am on October 12 when she was struck from behind by a car.
The 53-year-old was thrown three metres from her bike.
Witnesses came to her aid and called for an ambulance before she was rushed to Tamworth hospital.
Ms Lewis suffered a shattered tibia and fibula in her right leg, a fractured ankle and a compression fracture to her vertebrae in her spine.
She has since been released from hospital.
Last week, Ms Lewis made a public plea in The Leader for local cyclists to wear a helmet when out riding which she credits for “saving her life”.
