A helicopter landed at the scene of the crash to transport a man in his 50s who had been trapped in his vehicle with possible spinal and chest injuries.
Ambulance Victoria spokesman John Mullen said the man was in a serious but stable condition when he was transported to the Royal Melbourne Hospital.
The air ambulance arrived within 20 minutes after being called by local emergency services.
The incident occurred at the intersection of Suburban and Victoria streets around 1pm.
Three females from the other vehicle, understood to be in their 20s, were taken to the Ballarat Base Hospital.
All three were in a stable condition when taken from the scene.
The intersection is notorious for accidents with many locals saying they had seen numerous accidents occur over the past few years at the site.
A number of CFA units were also called to the accident.
