Just after 10.30am, police said a car travelling along Thunderbolts Way near Yarrowyck, appeared to cross to the wrong side of the road before hitting another vehicle head-on.
A 31-year-old man, with multiple fractures, was airlifted to Tamworth hospital by the Westpac Rescue helicopter.
Two children, in the same car, escaped uninjured but were transported to Armidale hospital as a precautionary measure.
A woman in the other car suffered minor injuries and was taken by ambulance to Armidale hospital.
The circumstances surrounding the crash are under investigation.
